  • Student Leave of Absence Form

    Please fill in this form to request the Headteacher of your child’s school to authorise a leave of absence during term time. There is no entitlement for any family holiday or leave of absence and the Headteacher will only consider the request in exceptional circumstances. Our school is following Southampton City Council’s commitment to raising attendance and will robustly challenge unauthorised absence.

  • Administration of Medicine

    The school does not hold a supply of pain relief etc. Parents may supply medication for their child, but the attached form has to be filled in correctly and signed - Please speak to Ms Lyons if you have any queries.

  • Confidential Medical Form

    Please use this form if your child is going out on a school trip. Also if there are any medical changes that the school needs to be informed of.

  • Parent opt in form for the Biometric Cashless Catering System
