Please complete this form to request the Headteacher of your child’s school to authorise a leave of absence during term time. There is no entitlement for any family holiday or leave of absence; therefore the Headteacher will only consider the request in exceptional circumstances. Our school is following Southampton City Council’s commitment to raising attendance and will robustly challenge unauthorised absence.

The school does not hold a supply of pain relief etc. Parents may supply medication for their child but the medication needs to be in the original packaging with the attached form filled in correctly and signed. Please speak to Ms Lyons or Mrs Graves if you have any queries.
